Access Medical, Inc. offers services throughout the country, and we are anticipating an exciting and successful year with increased expansion being a part of our goal. Our rapidly expanding reputation for quality, integrity, and professionalism, offers you over 100 various referral sources with the confidence of credible, well written reports by Board Certified, practicing physicians. Your presence and ability to perform Independent Medical Evaluations in your area of the country and beyond would be a key element in our ability to expand and offer increased services to our extensive client base.

Access Medical's goals include timely, quality service to our clients. Our service standards and procedures are as follows:

SERVICES PROVIDED TO OUR MEDICAL EXPERTS

 

  1. We will forward medical records to you, including a cover letter, within one week of the examination.

  2. All records will be in chronological order.

  3. Last minute cancellations will be charged to the client. You will receive your customary cancellation fee.

  4. Rescheduled examinations, due to cancellation, will be done in enough time so as not to block your calendar with wasted scheduling time.

  5. If the reserved time you have scheduled with Access Medical is not going to be filled, we will inform you within one week so this time is not wasted.

  6. Access Medical provides a convenient 24-hour, 7 day-a-week teledictation system.

  7. Payments will be made on the 30th of the month upon receipt of invoice. Invoices need to reach our office by the 23rd of the month to insure prompt payment of your billing.

QUALITY ASSURANCE

 

  1. Our Quality Assurance Department will contact you personally with any quality assurance questions or concerns. We would appreciate your prompt response so that our reports remain timely and accurate.

  2. Addendums or clarifications to original reports of previously asked questions are not charged to clients. Supplemental reports that entail answering additional questions or reviewing additional medical records will be charged to the client. Please inform us what your fee is for this service.

  3. In this competitive marketplace, timely reports are essential to ensure repeat business. Therefore, we ask you to please dictate your report within 24 hours after the examination is performed. This is helpful for you as well, in that the patient and your observations are still fresh in your mind. In addition, the following information should be standard at the beginning of the report: height, weight, age, general description of the patient and the complaint. Teledictation instructions are included. Please let us know how our system is working for you.

In order to expedite the scheduling of appointments, if you would be interested in working with us, we would appreciate the following information mailed to us as soon as possible:

 

  1. Signed W-9 tax form.
  2. A copy of your Curriculum Vitae.
  3. A copy of a sample report.
  4. Background Referral Information sheet.
  5. Proof of Malpractice Insurance
    (Copy of card or front page of policy)
